A completed copy of KOMP's Material Transfer Agreement (MTA) is required for all orders. A link to the correct form can be found at the bottom of the page, but please read the entire page for important information. If you need help choosing the right combination of MTAs for your order please contact us at service@komp.org

There are multiple ways in which you can return your completed MTA to us. For ES Cells, Mice, and Germplasm requests:
  1. A digital (scanned), signed copy can be uploaded directly to our website by clicking on your current order number on the order status page.
  2. A digital (scanned), signed copy can be emailed to orders@komp.org.
  3. A paper copy can be faxed to KOMP orders (530)757-3284.
  4. Or you may mail a copy to:

For Vector requests:
  1. A digital (scanned), signed copy can be emailed to komporders@chori.org.

Mutant ES Cells, Embryos, Sperm and Mice: Please be sure to complete page 3 of the MTA with the knock out gene you are ordering and the type of embodiment you are receiving for that knock out (i.e. ES Cells, mice, etc.) Here is an example of what your page 3 should look like:

Please reference your 4 digit order number (i.e. KO-1234) on the MTA

JM8 Parental Cells: Requests for the parental wild-type ES cell lines requires the JM8 Parental MTA, be completed prior to distribution of these cell lines.

Parental Sub-line Cells: Requests for parental sub-line ES cells requires the JM8 sub-line MTA for the N4, F6 and Agouti sub-lines, be completed prior to distribution of these cell lines.
